How to Apply

1. Complete the scholarship application form, indicating your contributions to the field, and what you might give back to the field by attending this meeting as a scholarship recipient.
2. Mail or fax the scholarship application form to Lea Rucker, Association Forum Foundation, 10 S.Riverside Plaza, Chicago, IL 60606, or fax to 312-924-7173 by the deadline June 9, 2008
3. Recipient will be notified by Forum.

Eligibility

Applicants must be Association Forum members.  Applications will be considered based on financial need.
